What we collect
Your name, phone number, preferred date and time, whether you are a new or returning patient, and when you agreed to this notice. We do not ask for the reason for your visit or any medical details.
Why we collect it
To arrange, confirm and follow up on your appointment.
Who can see it
Dr. Khemlani's office staff, the service providers used to run this website and booking system, and the calendar the office uses to schedule appointments.
How long we keep it
We keep your booking request for 30 days after you send it. If the date you asked for is later than that, we keep it until that date has passed. After that it is deleted automatically from our booking system. When your appointment is confirmed, it is also added to Dr. Khemlani's calendar, and that calendar entry stays there until the office removes it.
Where it is stored
Your booking details are stored on secure servers in the Americas region, which is outside Jamaica, provided by the service companies that run this website and booking system. When an appointment is confirmed, it is also stored in Dr. Khemlani's calendar, which is held by Google in its own data centres, which may also be outside Jamaica.
Your rights
You can ask to see, correct or delete your information, or withdraw your consent, by calling the office on (876) 428-6427.
Complaints
If you are unhappy with how your information is handled, you may contact the Office of the Information Commissioner of Jamaica.
